Determine Eligibility Criteria
Income-Based Eligibility: Household income must be at or below 135% of the Federal Poverty Guidelines.
Program-Based Eligibility: Participation in any of these programs qualifies:
Medicaid
Supplemental Nutrition Assistance Program (SNAP)
Supplemental Security Income (SSI)
Federal Public Housing Assistance (FPHA)
Veterans Pension and Survivors Benefit
Tribal Programs (BIA, Tribal TANF, FDPIR, etc.)

- Recent tax return or three consecutive pay stubs
- Unemployment or workers’ compensation statement
- Social Security benefits statement
- A benefits award letter
- A statement of benefits
- A letter of participation from the agency
